how do I find a severance payout statement from 15 years ago?

I received severance payout in 2004. how do I find out if a 1099 was filed and obtain a copy of that statement/letter and where do I begin to look?

If neither your lawyer nor the employer nor its insurance carrier (if it was covered) have it then chances are you won't.
